Abstract Adenocarcinoma of the third part of the duodenum is a rare occurrence. It is diagnosed at later stages and follows an aggressive course. We present a 42-year-old patient who came to us with symptoms of florid intestinal obstruction and weight loss. Upper Gastro-intestinal (GI) endoscopy showed circumferential growth in the third part of the duodenum with stenosis. Endoscopic biopsy was done which showed poorly differentiated adenocarcinoma. Computerised tomogram (CT) abdomen showed growth in 3rd and 4th portion of the duodenum with para-duodenal lymph nodes and multiple hepatic metastasis.
